This is in confirmation of my telephone call to your department today.

I mailed to you a formal release and request for records on June 30, 2003 to supplement my request sent on June 20, 2003.

Today Lee at your office indicated that my request was not in the computer and transferred me to your extension but you were not there but I left a message.

I am also faxing this further request.

I have attached the prior requests which are a part of this request.

As an aside I also requested copies of records from another hospital during this same period and when I called them today, they indicated they had sent the copies to me which I have not received.
