Re: Request for medical records/fees

To The Above:

In my telephone conference with David in Medical Records confirming that you received all four pages of my fax of today, he indicated that there was a retrieval fee of $13.00 and $00.55 per page copy charge to be made out to Correspondence Dept., Health Information Services, Mass Gen'l Hospital.

In order to not delay sending the copies to Anthem, I am enclosing a check in the amount of $25.00 which should more than cover the charges. Again if there is any overage and processing will cause delay of copies being sent to Anthem, please apply the balance to a pro bono/charitable purposes account especially if there are any for children receiving treatment at the hospital.
