I have received a letter from Anthem indicating that they have denied insurance coverage for most of the medical treatments and tests since the new sinus injury starting on April 18, 2003.

Evidently the denial includes both out of network and in network services and tests such as the CTscan performed at Cottage Hospital on June 9, 2003. The denial appears to even include my office appointment for my annual physical. When I received information of denial of coverage, I have also held off going to Cottage to get the blood tests and to take and send in the stool sample kit.

Please send copies of my medical records starting with my appointment with the physician's assistant, Ms. Brezina down to the office appointment for the annual physical. The copies of records are those covering the sinus injury, communications with, to and from other doctors, health care professionals, hospitals, surgeons, any other persons pertaining to the sinus infection and injury as well as copies of the annual physical examination.

Please send exact duplicates of the copies of the above records to me at the above address and to:
